Required Documentation

As a condition of employment, ALL candidates must provide documentation of the following:

  • School Personnel Health Record or Physical, must have been completed within 12 months of the

    hire date

  • Tuberculin or PPD Test, must be within three months of the hire date (the documentation is included within the physical form)

  • Drug Screen Test/Work Capacity Evaluation, must be completed by Greensboro Cornerstone Care

    • Work Capacity Evaluation, only applies to nutrition center and custodial/maintenance staff

  • Act 24 (PDE 6004 Form - Arrest/Conviction Report)

  • Act 168 (Sexual Misconduct/Abuse Disclosure Release)
